To expedite your first visit with us, please click the applicable link below and complete the registration over our secure HIPAA-compliant client portal before you and your child come in for your first visit. We have a portal for new clients and one for existing clients. If you choose to fill out the registration at our office, please arrive 10-15 minutes before your appointment to allow enough time to get them completed before your appointment.
